To make a referral for counselling please complete the form below. We can only accept referrals for young people in the Daventry District or Towcester areas, who are between the ages of 11-18. Or up to 25 if you have a diagnosed SEND
If you are completing the form for someone else, they must have given their consent and be fully aware of the referral. We reject referrals that do not meet our criteria. If in doubt call us on 01327 706706 to make a referral during our admin times. All data is kept in accordance with the latest legislation and in line with GDPR requirements. The data we collect is used for monitoring purposes only and on occasion used for anonymous case studies for quality assurance and contract review monitoring.
